Care Services for the Elderly, Blind and Disabled (formerly HCBS-EBD) are services provided in a home and community setting to individuals who are eligible for Medicaid reimbursement for long term care, who would require nursing facility care without the provisions of DCS-ESD services can be provided at no more than the cost of nursing facility care.

Personal Care Services
Services which are furnished to an eligible client in the clients home to meet the clients physical needs, when those services are not skilled personal care, or do not require the supervision of a nurse, and do not require physicians order.

Private Pay Program
SCCOG provides unskilled in-home care for Private Pay clients. Some tasks that we will help with are bathing, dressing, general household activities, meal preparation, medication reminding, companionship services, accompanying to physician visits or therapy.

Home Modification
Specific clients home setting that are necessary to ensure health, welfare and safety of the individual. This includes wheelchair ramps, bathroom modifications.

Non Medical Transportation
Transportation which enables eligible clients to gain personal physical access to non medical community services and resources, as required by the care plan.
